Forms and Record Request
There are many opportunities to become involved at Archbishop Carroll High School! Below you will find links to our interest forms for athletics, performing arts, and instrumental music. These forms are optional, but important if they want to receive the latest updates from coaches and program directors. Secondly, you will need to fill out the Records Request form so that we may obtain your student's final records at the end of their 8th grade year.
Finally, the World Language Placement Test form is for all students who would like to try to test into a higher level of Spanish. Please email bbranum@carrollhs.org with any questions about this test.

What is the “No contact period”?
During the period from January 16 until January 21, 2026, representatives from Archbishop Carroll High School cannot contact parents and/or students regarding admissions. This period of time is to allow families to discern their high school decision. However, parents may initiate contact with Archbishop Carroll High School during this time should they have any questions.
What is the next step to enroll at Archbishop Carroll High School?
In the acceptance packet you received in the mail, your student was given a list of recommended courses (green paper) for their freshman year, as well as an assigned day and time to meet with their high school counselor. On that day, as a family, you will meet with your student’s counselor and have a conversation about those recommendations, choosing classes for next year. We will also have someone available to help you complete your FACTS enrollment.
My student is very excited to come to Carroll. Can I buy spirit wear?
Your student will be given an Archbishop Carroll High School Class of 2030 t-shirt when they register for classes, as well as a sign for your family to proudly display in your yard. The Spirit Shop will be open during registration nights for all new families, and you will receive a 20% discount at that time. This is a great time to stock up on Archbishop Carroll High School gear!
