All Carroll students planning on participating in interscholastic athletics are required to complete online athletic forms through their FinalForms account.  Parents are also required to complete athletic forms through FinalForms.

FinalForms allows parents and student-athletes to complete athletic registration and athletic forms online.   Forms are electronically signed which eliminates the need to turn in paper copies.  Since data is saved from season to season and year to year, you are only required to electronically sign your forms once per year. (You will also be required to sign again following any updates made during the school year).  FinalForms will pre-populate information for additional Student-Athletes in your family.

The following athletic forms are mandatory before participating with an athletic team:

  • Athletic Physical: To print a physical form to take with you to your appointment, login to your student’s FinalForms account and click on the “Forms” tab.  On the left-hand side, click “OHSAA PPE Physical (for Physicians)” and print a copy of the physical exam form.  Pages 3 and 4 of the physical must be completed and signed by a physician.  These two pages are the only paper forms that are required to be turned in to the Carroll Athletic Office.  Physicals remain valid for one calendar year plus one month.  FinalForms will automatically send expiration reminders to the email addresses you provided.
  • Concussion Acknowledgement: Completed and electronically signed by both student and parent through FinalForms. You do not need to turn in a hard copy.
  • Sudden Cardiac Arrest Awareness: Completed and electronically signed by both student and parent through FinalForms. You do not need to turn in a hard copy.
  • Emergency Medical Authorization: Completed and electronically signed by both student and parent through FinalForms. You do not need to turn in a hard copy.
