Students planning on participating in interscholastic athletics are required to complete mandated athletic forms, along with their Parent/Guardian, through their FinalForms account. Students and Parents/Guardians are also required to electronically sign forms before participating.
An OHSAA Pre-Participation Physical Exam Form must be on file with the Athletic Office prior to participating with any sport. Physicals are valid for one calendar year, with a grace period of one month for insurance purposes. The physical form may be downloaded from a student’s FinalForms account under their “Forms” tab. The two pages of the physical form the doctor completes and signs are the only copies that need to be on file with the Athletic Office. These two pages may be uploaded to a student’s FinalForms account or turned in to the Athletic Office.
