  • I have read the description of the Diane A. Wourms Scholarship and detailed instructions.  I have provided information truthfully and to the best of my knowledge.  I understand that if I submit without all required materials or past the due date of December 1, 2022, my application will not be considered.

  • Please upload a copy of your school transcript.  Grade school transcript/grade report may be included if you believe they support your application.

    In addition, briefly describe an academic or personal struggle in which you have been confronted and upload this essay.  For example: a learning disability, an illness or disability, a personal loss, financial hardship, lack of direction, etc.

    Finally, in 400 words or less, write an essay or submit an artistic impression and upload this essay.  For example: a poem, drawing, model, sculpture, fabric art and/or photographic expression, etc. on one of the topics listed below:

    I see God’s hand at work through nature.

    How God helped me confront an obstacle in my life.

    Who is one teacher that has inspired you to be a better person, and how have they impacted your life?
